1) How long have you lived at this address?
2) When are you normally at home?
3) Who else lives on your property? Do you think they ever saw the animals on the neighboring property? What are their names and contact information?
4) Who is your neighbor (where the animals we're talking about are located)?
5) Do you know how to contact him or her? Her phone number, where she works, relatives' names and contact information?
6) Can you describe this person physically?
7) How long has he or she lived next to you?
8) Does anyone else live on the property, or visit your neighbor's property? (try to get names and contact information).
10) When did you first notice animals on the property? How many? What happened to each of the animals you've seen on the property?
12) How long have you seen the specific animal in question on the property?
13) Please describe the animal's physical condition when you first saw him or her and when you last saw him or her.
14) When did you first notice problems with the animals?
15) How long did the negligent care continue?
19) Describe any care you saw the animals receiving. Do you know who was responsible for providing care for the animals?
20) Did you ever observe the animals vocalizing or making noises? How often, when, circumstances, type of vocalization, etc
21) Do you think the owner could hear the animals vocalizing?
22) Did you ever observe the owner acknowledging the animals' vocalizing (by yelling at them, giving them food or water, etc.)?
23) Have you had any contact with your neighbor at any time regarding animals you have seen on her property? When? What type of contact? Specifically, did you ever point out to your neighbor that the animal did not look well? What was the response?
24) Has your neighbor, at any time, given you any indication that he or she was aware of the animals' physical condition, health, welfare or level of suffering? When, how..?
25) To your knowledge, has your neighbor made any statements indicating that she was withholding food, water and shelter from the animals for a specific reason?
26) Have you ever seen a veterinarian on the property examining or treating the animals? (who, when, etc)
27) Have you ever seen anyone giving the animals medical treatment or taking them to a veterinarian? (who, when, etc did they have the owner's permission.)
28) Have you ever given the animals food or water? Why? How often?
33) Please give us the names and contact information of anyone else who has seen animals on this property over the last year or have other information about the owner or her care of these animals.
